If you proceed with this assessment the following terms will apply:
a) This assessment is primarily based on your questionnaire responses and your self-report. However, in cases where the court order states that collateral third party information be obtained as part of the assessment, we will typically accommodate the court order, but limit the collateral information to one source (spouse, EX, Guardian Ad Litem, character reference, or a court document). If additional collateral information is requested your assessment fee may be increased according to the additional time needed or we may choose to refer you to a different assessment service.
b) The assessment report provided to you is a professional document that you and your attorney may present to the court if you wish, but due to the contentious nature of family court proceedings and frequent requests for second opinions, you agree that this is a non-refundable service in the event the court does not accept the assessment for any reason.
c) Your assessment will be conducted by an appropriately credentialed professional (evaluator, social worker or substance abuse professional). This assessment is not being conducted by a psychologist nor will it be considered a psychological or psychiatric evaluation. Your evaluator may be professionally licensed or credentialed in a different state than the court you might consider presenting the assessment to.
d) In the event that after submitting your assessment a court authority requires that we review additional documents or collateral information in consideration of further amendments to your report, you agree to pay for additional service time at the rate of $60 per 15-minute increment in advance of the delivery of any amended report or document.
e) In rare instances the evaluator may discover in the interview that his or her services are not appropriate or may not be helpful to you, therefore, the evaluator reserves the right to terminate the assessment service and ND Assessments will issue a full refund in the event that the evaluator believes you would be better served by a different provider.
