The goal of an alcohol assessment is to determine whether a clinical alcohol abuse disorder is present and whether treatment could help. You may be required to complete this type of evaluation in one of the following situations:

Request from a physician or treatment facility

Register for a secure account account online, call us at 1-800-671-8589, or register at one of our kiosk locations
Complete the basic alcohol assessment questionnaire online
Complete a 45- to 55-minute interview with one of our professional evaluators via phone or online video call
Receive your report by email to review and approve before forwarding or releasing it to authorities (typically within five business days—rush service available)
*Note: If you need to provide us with important documents from the court you may email, fax, or upload them to your account at any time.
You can start your evaluation online in just a few clicks, or head to one of our many kiosk sites to register for an assessment. All your data is stored securely on encrypted servers. Once your assessment process is complete, your evaluator will e-mail you a report within five business days. Rush processing service is also available for an additional fee.
All of our evaluators are qualified professionals who are permitted to evaluate, diagnose, and treat alcohol abuse and other disorders. Plus, our online assessment1 services are accepted in 48 states, including the California courts, probation services, and DMV.
