  • A request by a physician who is concerned about abuse of street drugs or prescription medication

  • When you or your family members are concerned about your use of drugs and/or alcohol and want to determine whether treatment is necessary

How to Request Your Drug or Alcohol Evaluation

  • Register for a secure account online, call us at 1-800-671-8589, or register at one of our kiosk locations

  • Complete the initial questionnaire online

  • Complete a 45- to 55-minute interview with one of our professional evaluators via phone or online video call

  • Forward any supporting documents (such as drug test results) to your evaluator

  • Receive your report by email to review and approve before forwarding or releasing it to authorities (typically within five business days—rush service available)

*Note: While reviewing your case, we may determine that completing a drug test with a lab may help you. You can find walk-in services near you if this is the case.
