How to Request Your Employer-Required Drug or Alcohol Evaluation

  • Register for a secure account online, call us at 1-800-671-8589, or register at one of our kiosk locations

  • Complete the initial questionnaire online

  • Complete a 45- to 55-minute interview with one of our professional evaluators via phone or online video call

  • Receive your report by email to review and approve before forwarding or releasing it to authorities (typically within five business days—rush service available)

*Note: If you are a Department of Transportation (DoT) employee, guidelines require you to complete an in-person evaluation only.
